QuarkNet acceptance AND CONDUCT AGREEMENT

I                                                           am planning to attend the 2001 Indiana University QuarkNet summer workshop.

 

THE FOLLowing REGULATIONS SHOULD BE CAREFULLY READ AND UNDERSTOOD BY BOTH THE STUDENT AND HIS/HER PARENTS. THIS FORM SHOULD THEN BE SIGNED AND RETURNED WITH OTHER RECISTRATION MATERIAL.

1.      All use of tobacco alcohol, and other illegal drugs are strictly prohibited.  Possession and/or use of tobacco, alcohol, or other illegal drugs will result in immediate dismissal from the QuarkNet workshop.

2.      Boys will NOT be permitted in the girls' dormitory wing, and vice versa, without the express consent of the QuarkNet counselors.

3.      QuarkNet students staying in the dorm must be in their assigned room for evening check by 10:00 P.M. each night, unless they are signed out in advance for a special activity.  After evening check, students may not leave the dorm without the express consent of a QuarkNet counselor.

4.      QuarkNet students staying in the dorms may not leave the city limits of Bloomington at any time during the Quarknet workshop WITHOUT PERMISSION OF PARENTS or the QUARKNET DIRECTOR, except for QuarkNet sponsored and supervised trips and events.

5.      QuarkNet students staying in the dorms will not be permitted to operate a motor vehicle while attending the QuarkNet workshop. Indiana University assumes no liability in case of an accident involving non‑university vehicles. Indiana University assumes no liability in case of theft or loss of non-university vehicles.

6.      Television sets are not allowed in the dorm rooms.

7.      QuarkNet students are expected to behave in a mature and responsible manner. Students that damage Indiana University property will be subject to disciplinary action, and parents will be held financially responsible for the damage.

8.      My parents/guardians and I release Indiana University and its agents/employees and the IU QuarkNet Organization and its agents/employees from any and all claims, demands and causes of action on account of any injury, illness or loss which may occur during my participation in the QuarkNet workshop, June 10-June 16, 2001.

 

DISCIPLINARY ACTION RESULTING IN IMMEDIATE DISMISSAL FROM THE QuarkNet workshop WILL BE TAKEN BY THE QUARKNET DIRECTOR IN CASES OF NON-ComPLIANCE WITH Quarknet workshop AND DORMITORY REGULATIONS.
